a. I/We hereby declare, on my behalf and on behalf of the person(s) proposed to be insured, that the abovementioned statements, answers and/ or particulars given by me are true and complete, in all respects, to the best of my knowledge and that I am authorized to propose on behalf of all the person(s) proposed to be insured.
b. I/We understand that the information provided by me/us will form the basis of the contract of insurance, is subject to the Board approved underwriting policy of the Company and that the policy will come into force only after full payment of the premium is made by me.
c. I/We further declare that I will notify, in writing, the Company of any change occurring in the occupation or general health of the person(s) proposed to be insured after this proposal has been submitted but before communication of risk acceptance by the Company.
d. I/We agree and consent to the Company seeking medical information from any doctor or hospital, who/ which has, at any point of time, attended to the person(s) to be insured, or from any past or present employer concerning anything which affects the physical or mental health of the person(s) to be insured and also to seeking information from any insurance company to which an application for like purpose has been made, for the purpose of underwriting this proposal and/ or claim settlement.
e. I/We authorize the Company to share information pertaining to my proposal, including the corresponding medical records, for the sole purpose of underwriting and/ or claim settlement and with any governmental and/ or regulatory authority.
f. I/We hereby declare, on my behalf and on behalf of the person(s) proposed to be insured, that I have fully understood the product features, including its suitability, the contents of this Proposal Form, and all other connected documents significant and incidental to availing an insurance policy from the Company.
g. I/We agree to receive service related information from Edelweiss General Insurance Co. Ltd. and its service providers from time to time, through electronic and telecommunication mode including WhatsApp, and understand that no unsolicited information will be sent to me/us.
h. I/We state that the salient features and terms and conditions of the proposed insurance contract have been explained to me/us in vernacular language, and I/We agree to the same.
i. I/We confirm that the premium amount(s) being/to be paid against/under this proposal is/shall be from bona fide sources and not in violation of the provisions of the PML Act, 2002, and its amendments. I/We agree to fully cooperate with the company if document(s) to establish source of such premium amount(s) is/are called for, and understand that the company possesses absolute and sole discretion to cancel the policy, as and if issued, basis my/our conviction by a court of law or competent authority under/relating to the PML Act, 2002 and its amendments.

What Kind of Cover Can I Expect?
When you pick the Arogya Sanjeevani policy, you and your family members will be covered for:












For ICU and ICCU charges, we’ll provide up to 5% of your sum insured, subject to a limit of INR 10,000/- per day.






What’s not covered
There are a few things that are out of our hands. This policy will not cover*:
- Expenses related to any admission primarily for diagnostics and evaluation purposes.
- Procedures and medication related to obesity and weight control
- Treatments to change gender
- Cosmetic or plastic surgery, except when required medically to remove a direct health risk or after an accident
- Cost of care provided at home or in the out-patient department
- Expenses related to the treatment of injuries caused by participating in adventure sports
- Cost of treatments required after the insured individual commits or attempts to commit an act that breaches the law
- Treatment of alcoholism or any substance abuse
- Cost of dietary supplements and unproven treatments
- Expenses related to the treatment of sterility and infertility
- The cost of childbirth, including complicated and caesarean deliveries, except in the case of ectopic pregnancies
- Treatment administered outside the geographical extent of India
- Cost of treatment required in war and war-like situations or after a nuclear, chemical or biological attack

has a waiting period during which time certain claims will not be entertained. The waiting period for various issues is as follows:1. 30 Days – For all claims except those that arise as the result of an accident
2. 24 Months – For specific conditions, surgeries and treatments, including, but not limited to, benign tumours, gastric ulcers, non-infective arthritis, varicose veins and congenital anomalies
